What to expect
To ensure your application process is as easy as possible, we recommend you:
- Allow approximately 15-30 minutes to complete this form.
- This form asks for information about the person that needs services. Have their details and documents prepared and within easy reach. As part of the application, you may need to provide the person's NDIS participant number and plan, private health insurance details, GP referral, diagnostic reports or legal documentation relating to any legal orders that may be in place.
- If you need help from someone, please ask them to sit with you and/or call Therapy Focus.
- The form is not submitted until the 'Submit' button is clicked on the last page.
- If you are unable to fill out the form entirely, you can 'Save and Continue Later' by pressing the button at the bottom.
